Annual Health and Medical Form (AHMR)

• Parts A and B are needed for all short-term events (less than 72 hours), while part C with medical exam is needed for over 72 hours, such as Summer Camp, Wright-Patt, or High Adventure. 

• Parts A and B of the new form are to be completed annually by all BSA unit members and anyone attending a troop overnight event, and cover all events under 72 hours. Medical information required includes a current health history and list of medications.

• Part C is required along with parts A and B for any event that exceeds 72 consecutive hours, such as Summer Camp, Wright-Patt, or High Adventure. For additional information and electronic copy of the form, please visit the Scouting Safely General Resources page.
